Parental Consent: In the event that my son/daughter is injured whilst playing/travelling to or from football events for Failsworth Dynamos JFC teams and I cannot be contacted on the above numbers, I hereby give my consent for my child to receive medical attendance.
Please note that we would request Parent’s permission to occasionally use team and individual photographs to promote the club both internally and externally. Photographs will only ever be taken by club officials or professional photographers and we will always comply with the FA’s guidelines on child protection. If you would not like your child’s photo used in this way, please contact the clubs Child Protection & Welfare Representative – Claire Dalton on 07496 864907.
As a parent/guardian of the above-named player I undertake to pay Failsworth Dynamos JFC the annual membership fee of £25 and to pay the monthly subscription(12 MONTHS) of £25 by setting up a standing order for the 1st of every month. Payments missed for 1 month will incur a player suspension until cleared.All fines incurred by players are their responsibility to pay,not Failsworth Dynamos JFC
All kit supplied remains the property of Failsworth Dynamos JFC and must be returned to the club should a player leave. All debt incurred must be repaid before the player will be released to join another club/team.
Whether you are a club official, player, parent or a spectator we have to abide by the Club constitution, rules & codes of conduct (these can be found on our website).
By registering your son/daughter as a player for the Club and by signing the above declaration you are agreeing to abide by the said constitution, rules & codes of conduct of the Club.
